SUMPRODUCT? COUNTIFS? Extract data with multiple critea

I am trying to figure out how to extract data from 4 columns into one table... I need to list the customer and status by month and year. Pending status has IFERROR (1/1/0001)... Approved Status has two good dates. I sont know if there is a way to put all of this in one table (multiple customers, and their status with month and year) so I started with what I thought would be a simple table for me to understand. I hope all this makes sense.

Any help you could provide me would be greatly appreciated.

I am looking to have the customer info show in table format by month and year... (much like what is in the DATA tab). I need the total amount of "Pending" or "Approved" by month and year.

protonLeah, I just realized the formula enclosed in your file does not populate what I am in search of. Thank you for trying to assist me. I am still in search of a solution.
