need to change format and concatenate 4 column to one

Hi everyone
i need any your help i have a data of attendance hr fill it on excel
and need to change format all data and export to text file for use on another program
here is a exsample of need format
ID Date Time Status Format
5709853 1/4/2016 07:46 I 5709853,01042016,0746I
5709804 1/4/2016 07:47 I 5709804,01042016,0747I
