Conditional Formatting not working as expected

My workbook uses conditional formatting rules to color cells background depending on values in other cells, however some cells show a black background - The conditional format rules do not select that coloring.

The original workbook is very large and I couldn't attach it so I made a smaller version (attached). There are many invalid references with that smaller version but the conditional formatting problem is there as well.

The offending cells are:

U38:Z39
U41:Z42
AD38:AL39

I appreciate any help to resolve.

Those cells have been manually coloured.
Highlight them all, right click and select "No Fill".
