index match with date range help!

I have a table I am trying to pull info from for another table and I think the index match function is the way to do it but I am having trouble with the formula. what I would like to accomplish is for the respected unique identifier ("BASE") to populate the coupon if the maturity is in the respected range above. Hopefully someone can easily help out here

So if you look at the workbook, the Raw data is on the “Download(2)” tab. On the Scrub tab I would like to match the respected BASE identifier and pull the coupon value if the maturity (from the raw data) is within the date range in row 2 & 3 for the respected column. So you will see BASE “66612A“ has multiple maturities (5/12/17 and 6/10/17) and the function will ideally populate 0.55 in cell B5, 0.80 in C5, and the other colunms will be 0.
