lookup year and month to return value

Hoping an easy solution someone can help me with. Attached is my milage form and the problem I have is im using a pivot table to return a milage figure for each month. All has been good until this year ive reached Jan again as a pivot table only returns the first result location which for me is 2018. I need my attached form to look at the year, then month and return the value in the summary sheet column "H". Can someone help.
ive made some notes on the attached excel sheet on each tab. The form style I cannot change as this has to be submitted as is

Just remember ( that is not always clear in the explanations on the Net), that INDEX on its own returns the value of a cell. Embedded in a formula it returns the cell reference to the formula enabling you to build interesting formulas
