Read a cell value from the cluster of small tables

The idea is to get the SCORE risk value for a patient, based on attached charts, knowing the input values: age, ***, smoking status, cholesterol and systolic blood pressure. Input values would be typed in in the cells A1:A5 and A6 should show the score.

    Welcome to the forum

Please attach a sample workbook (not a picture or pasted copy). Make sure there is just enough data to demonstrate your need. Include a BEFORE sheet and an AFTER sheet in the workbook if needed to show the process you're trying to complete or automate. Make sure your desired results are shown, mock them up manually if necessary.

    Remember to desensitize the data.

Here is the mock-up workbook. In the SCORE sheet i need to get the SCORE value (red). The value should be read from the tables on Men and Woman sheets, based on inputs from rows 3-7. So far I tried with large IF-THEN formulas, but its too complicated. Hope ther is a easier way.

This is not what I would consider an easy task even with a row over row (a.k.a. proper data set) arrangement.
    1. Arrange the data row over row as modeled on the Patient sheet.
    2. Produce ascending tables for BP and age
    3. The formula that populates A9 (as modeled in cell A10) is:
    Formula: copy to clipboard
