Formula/Conditional Formatting error questions

G11, G12, and G13 I would like to correlate with D12's values.

D12 (0-14) turn G11 Green with conditional format and text prompt via formula.
D12 (15-29) Turn G13 Orange with conditional format and text prompt via formula.
{example: =IF(AND(30<=D12),"We have reached criteria for RED level interventions. Please implement RED level interventions.","") }
D12 (30+) Turn G15 Red with conditional format and text prompt via formula.

D12 is changing colors accordingly, but G11,13,15 are not and I cannot seem to figure out why.

Also, H18:21 (comments). I would like to dates to automatically display today's date when anything is typed into these rows, showing no date if empty. I cannot get these to change accordingly. Example (=IF(H18<>"",IF(CG18="",TODAY(),H18),""))

I found this version within a forum online. I've gotten it to work on a separate sheet, but not on this. I am also unaware of where/why the "C" is included in the "CG18".
