    Two days and cannot figure out INDEX/MATCH solution

    I am attempting to return the value of other columns much like a VLOOKUP would; however, I need to find the applicable row/column to ensure a match and then if true return data from another column on that row and if there is no match I need to indicate as such. Attached is an example of what I have losing sleep over.

    A picture is not much use to us, especially one which can't be read very easily. It would be better if you attached a sample Excel workbook, by following the guidelines given in the yellow banner at the top of the screen.

    The attached might possibly do the trick- there are probably better approaches out there, however.

    Excel 365 user. To unblock a downloaded macro-enabled workbook, go to your "Downloads" folder > right click on the workbook name > click 'Properties' > check the 'Unblock' checkbox. You can now open the workbook.
