VLookup Not Working with Drop Down List

    Hello,

I'm trying to make a PO form for our fire department and it's not quite working. I have a drop down list in Account Name. I want that list to return lines for budget number, project code, and project title from the FD Project Code Data tab. I can either get the wrong information, or no information, but I can't get the right information, but I think I'm doing everything correctly. I'm sure I'm missing something simple, can someone look over this and let me know your thoughts?

    Thanks!
