    Using Index & Match to search 2 vertical columns and return data to a vertical set

    Hi, I am desperately in need of help! This has been troubling me for far too long!

    Basically I have a set of data containing multiple student entries. Each row represents a student and a new subject with a grade. i.e:

    Name - Subject - Grade
    Student A - Subject 1 - Grade 1
    Student A - Subject 2 - Grade 2
    Student A - Subject 3 - Grade 3
    Student B - Subject 1 - Grade 1
    Student B - Subject 2 - Grade 2
    Student B - Subject 3 - Grade 3
    Student B - Subject 4 - Grade 4

    I need to convert this to a horizontal format such as this:

    Name - Subject 1 Grade - Subject 2 Grade - Subject 3 Grade
    Student A -
    Student B -
    Student C -
    Student D -

    I've attached a template. Please if anyone can assist me I'd really appreciate it! I've made some progress using the following formula:

    =IFERROR(INDEX($G$2:$G$1216,MATCH($K2,$D$2:$D$1216,0)*MATCH(C$1,$F$2:$F$12160)),"")

    But can't figure out how to get it to work!
