Help with Vlookup formula from the option menue

Hi
Can someone help me with the following please:
I want to build a form that will mainly be use by managers to input the delegates names and choose from drop down menu the type of Replacement for an employee.
I have attached file for better understanding of what I would like to achieve.
Column F contains the criteria for the data validation drop down menu and column G contains rates, I want a Managers to choose delegate name and then choose whether a person needs to be replaced for the duration of the training and if so they can choose from drop down menu the type of the replacement which will bring appropiet value in "Training cost report" column C.
I would be very grateful for any help as I struggle with this for the last few days.
