Find relevant data and show in cell

Hi,
I have attached a sample workbook which I need to look @ the data in worksheet Data and paste into and if a corresponding magic number is in the sheet Reminders show it in n to r depending on the contact date. I have also on occasion duplicates in sheet Reminders, and require all entries after that date to go into the duplicate row, Is this possible to do and if so how would I do it?
