Formula populates correct info on original sheet & incorrect info on duplicate 2nd sheet.

    Formula populates correct info on original sheet & incorrect info on duplicate 2nd sheet.

    Click on the "Sequoia" tab and you will see in column "W" a formula that works well with column "U" and "V". Click on Sequoia(2) and you will see the same formula in column W however on this sheet it does not work. If there are not 8 patients to alphabetize the formula duplicates the first patient in the list enough times so that the total populated cells always = 8. I have been working on this all day, and I am sure it has something to do with a fried brain. What am I missing here?
