Auto Populate a Date based on a grading scale

I'm creating a spreadsheet for tracking my employees performance. I have a review sheet where they get a total of 1-5 on 6 different categories and then a total score. Depending on there score, they will have another evaluation "x"days after. An example would be a score of 5=60days out from the review date , 4-4.9=45days, 3-3.9=30days, 2-2.9=14days. How do I formulate the cells so that when I get the score of the review in then it automatically calculates the next review date based on the scoring guide. Any help is appreciated. Thank you!
