It's a bit ugly, 'cos of the data layout (much easier with one block of data, rather than 4). However:
=INDEX($D$7:$AC$9,MATCH(C12,$C$7:$C$9,0),MATCH(E12,$C$5:$AB$5,0)+MATCH(D12,$D$6:$H$6,0)-1)
