If MATCH with an OR qualifier?

So I am trying to test if a name is on a list on another sheet, or if the name also appears in the data set above the current row. If either are true, then the formula should return "Y", if not it should return "N". This is my attempt, but it's not working:

This attempt seems to work if the name also appears in column D above the current row, but it does not seem to pick up names that could also be on the Customer sheet.

Any ideas?

To attach an Excel workbook, click on GO ADVANCED and then scroll down to Manage Attachments to open the upload window.
