Index Match with Multiple Criteria

Hi,

I'm trying to perform an index/match function with multiple criteria.

The data I need to reference will be in the "Source Data" tab.
The report I'd like to automate is the "Tally" tab, in the red boxes (I manually added the values in red text, so you know what I'm hoping to see).

In cell D3, I tried using the formula =INDEX('Source Data'!A3:F14,MATCH($A3,'Source Data'!A:A,0),MATCH(D$1,'Source Data'!3:3)) but it returns an incorrect value.

Do I need to use an offset?

Any help would be greatly appreciated! Thanks!
