Form Data Posting Outside of Table

Hello,
I've set-up a table and I'm attempting to use a Form to enter data. The first entry is fine - populates the first row on the table, the second entry populates on the first row OUTSIDE the table and the next entry attempt results in "Cannot extend table or database" which makes sense as I'm now outside the table.
I've searched and searched, thought maybe my copied and pasted file was corrupted so I started a new one - same thing - I'm consistent.

Thanks in advance for any help but please keep it simple - after 20 years, my computer skills are pretty much limited to data entry
