Cannot set an individual link from a list item

Can the expert's on here advise me on something i thought may have been easy but I was wrong. I have created a list (data validation) is there anyway of linking every item in the list to an individual file as at the moment when i select the first item it goes to the file but when i select the next item it goes to the same file or is there a way of selecting an item in the list so it opens a comment box the problem is i dont want to use any macros. the idea is that once an item is selected in the list it brings up something that contains text whether it be a word text file or a comment box. Any help or advise would be appreciated.

Are you able to upload a sample file. Fast answers need clear examples. Post a small Excel sheet (not a picture) showing realistic & representative sample data WITHOUT confidential information (10-20 rows, not thousands...) and some manually calculated results. I dont have any file as i have not started yest was just testing the water if I did upload a file all it would have is an excel column with a data validation
list .There is 3 items in the list what im looking for is a way to click on an item in that list that either opens a comments box or goes to a file when i have tried it
evry item goes to the same file as if it is one link sorry if i have not explained to well
