    Red face Capture Data From User Form to Excel

    Hi

    I am creating a data capture form. But I am stuck at couple of things. I have created a combo box. What i want if i select 1st value then it will show me the value Ravi Kumar in TextBox2 and if I select 2nd value in Combo Box then it show Gaurav Sarin. But it is throwing me an elseif error. Also I want to use a date function in that form.
    I would be very grateful if someone can help me out. I have also attached the workbook.
