Need a form that ensures all fields are filled in and an email is sent to an email address

The form needs to have fields that are both either free form and drop down lists. When the form is completed, all fields must be filled in and then a button needs to be clicked to send the form to a specific email address.

I was able to produce something like this years ago, but I am completely out of the loop. I attached a sample form which needs the functionality present.
