Inserting multiple Yes No Radio Button into a Form

Hello, I am hoping I may gain assistance with this question.
I have a form with 40 questions all which have a yes/no answer.
I am wanting to create multiple yes/no buttons so each row (question) provides a number answer either a 0-1 or a 1-2.
Can this be done using VBA code, I have been unsuccessful trying to create rows of radio buttons as they all have linked to one cell.

If you are using Form controls on the worksheet you can add Frame control to have separate groups of controls.

If you are using ActiveX controls on the worksheet you can set the GroupName property
