Could any one help me here?

I have been working on customizing a userform that acts more like a query form and can generate reports based on the options selected in the field. Please find attached a sample report. I would like to know if it is possible to make dynamic selections using dependent combo boxes and get a report through the user form on a seperate worksheet within the same workbook. PFA an excel sheet which gives you an overview of the data that I have which is quite extensive.

Would appreciate if someone could give me an outline in starting with this!
