Please help, filling different section of one word using form drop down dynamic selection

I have the attached excel file in it, i want to use a form to fill out different section of one word in one cell, you will have a better idea once you see it, but basically i want to use dynamic drop down combo boxes in a form, i know you can use data validation and multiple drop down list but i need it to be used within a form. please help.

im trying to add this but its not working please add to example if you can, thank you. also i think if there is add button on the form it would make more sense, sorry.
