Custom input box (userform)

Hi I have multiscreen setup and the standard msgbox command dosnt always center on my main screen. Its my understanding that the only workaround for this is to create a userform

I almost have it. But im stuck on using it in a sub. Here is what I got:

But the problem im having is to use this in another sub say something like this:

Im not sure how I can halt the test sub to wait while I input value from the checkbox in the userform and press "OK" btn.
Anyone have some pointers to make this usable ?
