Getting Label Caption Value And Insert them to specific sheet with Submit Button

ok, I just created a new account only for this and please bear with me because I'm really new to VB-Excel programming.

So here is my problem:

I want to make simple personal money tracking that I can track and allocate spending easily with Excel+VB. I somehow already managed to create Userform and finally I'm currently stuck on submit button part.

Basically what I'm trying to do with the button is to reference value from the label caption above it (in the same userform), and records those value to new row in specific sheet.

Here some screencap for clarity of my problem.

So from that pic I need to get those value in red and records it to the green marked sheet.

I will really gratful if someone here kind enough to give me guidance for my problem.

Attaching a workbook will be better than a screenshot.

1. Make sure that your sample data are REPRESENTATIVE of your real data. The use of unrepresentative data is very frustrating and can lead to long delays in reaching a solution.

2. Make sure that your desired results are also shown (mock up the results manually).

3. Make sure that all confidential data is removed or replaced with dummy data first (e.g. names, addresses, E-mails, etc.).

4. Try to avoid using merged cells as they cause lots of problems.

Unfortunately the attachment icon doesn't work at the moment, so to attach an Excel file you have to do the following: just before posting, scroll down to Go Advanced and then scroll down to Manage Attachments. Now follow the instructions at the top of that screen.

Please pay particular attention to point 2 (above): without an idea of your intended outcomes, it is often very difficult to offer appropriate advice.

Thank you for your input. I already uploaded the workbook and mockup the result that I desired on "Input-History" Sheet.

edit: I can't seem to reupload my workbook right now. the workbook that I attach on my first post have some mistake on "timestamp" column. the timestamps should be the same instead.
