VLookup for dates of payments for every person.

I need to make a chart that represents when payments occurred. This payment plan has 7 payments.

So I need a way to say that John Smith made his first payment on June 1, his second payment on July 1, and so on. But I need to have hundreds of names on the same sheet. Any help??

Basically Cell 1 is John Smith, cell 2 is date of 1st payment, cell 3 is date of 2nd payment and so on.

I've attached a data sheet. That has name and date, hopefully it gives you an idea of what I am looking for. Sheet 2 kind of represents my goal.

The Effective date may have duplicate dates for Rep 1, so if we can combine the payment amount for the same dates?
