    Same Pivot Table Elements have different format when using Custom Pivot Table Style

    I am currently using Microsoft 365, and I have a Pivot Table that I'm trying to format using a custom Pivot Table Style. I have attached a screenshot of a portion of this table (see below).

    I do not understand why the row labeled "Physician 2" is not formatted the same as the row labeled "Physician 1" (my desired format). Physician 2 has a smaller font, and the totals are in black (not white).
    Both physicians should be the SAME element (Row Subheading 1) and, therefore, should have the same formatting... right? What did I do wrong?

    My thought is that Dr. Joe Brus is the column header of the source data.
    If that is correct then that may be why that row is formatted differently.
    If the column header of the source data was say "Physician" then I would expect the Dr. Joe Brus row and the Dallas Medical Center row to format the same.
