Unique count with multiple criteria-Solved

Good day,
I am trying to determine the fail percentage for unique values and am stumbling on including the multiple criteria across the columns.
I need the count of unique members with unique fail reasons per term so I can use that to determine a percentage. I can find the unique team members in the list, but am hung up on counting fail reason for unique terms per member, is there some way to embed an if into my unique team members formula or a better way to do this? I have attached the sheet I am working from for reference.
