Multiple Ifs statment to indicate True or False - Only 1 of 5 rows is True

Good morning,

I have an account that is in five different composites at different time range 2012-2017 (DATEOVER_Entry and DATEOVER_Exit 1 to 5). I want to create a formula to say if the Adate Nov-13 (in column C) is either:

True "it is in GIPSMOD (column B) because it is in that DATEOVER_Entry01 and DATEOVER_Exit01 (there are five DATEOVER Range) or
False "It is not in the composite"

In my example, the Adate (Nov-13) in column C is the month that has a value difference and I only want to identify "YES" the GIPSMOD (column B) because the Adate (Nov-13) in column C is within the DATEOVER01_Entry and DATEOVER01_Exit range.

It is a little difficult to explain and hope you understand my question. Thank you in advance for your help.
