Conditional Formatting to color-code 1 cell based on data in another field

I need to color-code cells in the same column based on response in another column on the same row. For example:

If column Y = A, column B needs to show a blue background - existing condition
If column Y = B, column B needs to show a red background - new condition
If column Y = C, column B needs to show an orange background - new condition

And a fourth rule:

If column X = Yes, column A needs to show a green background - existing condition

I'm not sure what I'm doing wrong but when I added the additional 2 color-coding conditions, my formatting went awry. Any assistance appreciated.
