Point System Attendance - Pivot Table Assistance

Hello,

My company will be going to an 8 point attendance system come January 2014.

I am trying to create an excel sheet that will facilitate this for me. My question is, how can I generate a report to see by employee the month the infraction took place?

I saw something similar on a different excel sheet that reports codes versus points (see picture).

I was unable to duplicate it since I am not familiar with pivot tables at all.

Can someone assist me with this?
