creating a web form with selection to input into a spreadsheet.

Hi,

I am trying to create a web form to have multiple users enter their performance with respect to their equipment into a spreadsheet. As I have almost 'zero' knowledge in web form creation, thus I am seeking help.

In my spreadsheet, the equipment is identified by LT1, LT2..... and the users are to key in their column C,D and E with respect to their equipment.
