data auto fill base on condition.

Hi,what i want is in dashboard sheet i will change month, date & operator. so that details need match with my second sheet (data) & need add that matching details to dashboard sheet row 12 to down ( pls my sample sheet. i have manually added one data line) so pls give me a vba macro code to do it. thanks in advance.
