Survey creating(new to coding etc.)

I am trying to create a analyse a survey, I have 5 radio boxes all the way along with a 10 question questionnaire, they all generate a score from 1-5. I would like it to collate all the scores from one survey and with use of a command button it put all the scores onto a new sheet and clear the spreadsheet.

Any help would be massively appreciated!
